How do i train my puppy?

Shes so tiny and she doesn't really "tend" to food or toys, she also runs off in the yard and she is scared of her own leash but loves to run around, She also has a nipping & barking problem, along with NOT housebroken, doesn't eat her dog food, and she is only 4 months,PLEASE help!!!!!
